Medical Release Forms and Other Team Information

Hey this is a problem pretty frequently…so lets have a look at it.

1st - This information is not on the vcaa.org site…it is currently held in the CMI Registration servers so in order to access it you will need to log in to the CMI servers using your Username and Password. Here is a link to the Log In.

2nd - find the team tab at the top of the page, here is an example

3rd - select this tab and you should see a list of the divisions  you are associated with, select the division of interest.

4th - you should then see a list of teams you are associated with , if you do not see the team you are looking for please be certain that you have volunteered for the league and you have been “approved”.  If you are certain that all those criteria have been met then contact you division commissioner who will in turn be able contact the correct administrator to solve the problem.

5th  - given all things are working select the divison you need to operate on and you will be presented with a team info page …like this

6th - Select the  team in question and you will get another page with even more team details…like this

Select the Roster Tab.
